- Respond to our requests for information (such as vehicle or driver information)
- Follow up with the primary care doctor to make sure they are responding to our requests for information
- Log into this website or call us the business day before the trip to get the pick-up information
Things we may need before a trip can be scheduled:
- A referral from the primary care doctor
- Documentation showing why the local transit or a personal vehicle cannot be used
- Up-to-date and valid:
- Driver's license
- Vehicle registration signed by the owner
- Proof of insurance
